Our Community Healthcare Professionals has three core internal organizations: Community Hospice, Community Home Health, and Community Personal Care. Each of our internal organizations provides its own unique services, headed by directors who are deeply invested in the success of their department. We invite you to learn more about each department and the services they offer below.
Community Hospice Care consists of Physicians, Nurse Practitioners, RNs, Chaplains, Social Workers and Volunteers. Our team specializes in pain management and symptom control. We involve all family members in our care planning process.
Community Home Health & Extended Care provides nursing and therapy services on a short-term basis. Our nursing staff specializes in Pediatric Services. We also implement a transitional program for those approaching end-of-life care.
Community Personal Care sitters assist with care on all levels. Bathing, cooking, shopping, cleaning, and companionship. While family and friends often want to be there to help loved ones recover, this isn’t always possible. This is where we help.
Our goal is to deliver the best care for our patients – whether they need home health, private duty, palliative, hospice, or personal care services.
