Home Health & Extended Care
Community Home Health Care provides nursing and therapy services on a short-term basis. Our nursing staff specializes in Pediatric Services. We also implement a transitional program for those approaching end of life care.
Hospice Care
Community Hospice and Palliative Care consists of a Physician, Nurse Practitioner, RN, Chaplain, Social Worker and Volunteers. Our team specializes in pain management and symptom control. We involve all family members in our care planning process.
