Our Director:

Rosalyn Collins, RN is the Community Group’s Director for Home Health and Extended Care Services. Rosalyn has been in this position for 6 years. Prior to assuming the position of DON Rosalyn spent 4 years as a Home Health case manager in the Home Health Care setting. Rosalyn obtained her LPN certification from Louisiana Technical College- T.H. Harris Campus in 2001 and Received her RN Degree from LSU Alexandria in 2007. Rosalyn currently resides in Opelousas Louisiana with her Husband Daryl, as well as her three wonderful children Alexandra, Khalid, and Ashlyn; as well as her 1 grandchild Legend Mayer

Community Extended Care Services

Is your child (or patient) in need of a nurse to care for him or her in their home?

If so, he or she may qualify for Skilled Nursing (also known as Extended Care Home Health/Private Duty). This service is provided through private insurance, Medicaid, and/or private pay and it meets the needs of individuals with developmental/mental disabilities and/or those in need of medical care on a daily basis.

  • Providing competent & trained Licensed Practical Nurses (LPN)

  • Meeting your child’s physical, developmental, and emotional needs

Private Duty Nursing

Our private duty nurses are available whenever you need them. From scheduled nursing visits to hourly care, someone is there to help 24 hours a day, seven days a week. We work with your loved one’s medical team to offer the professional support he or she needs.Private duty nurses allow for loved ones to live at home while receiving the care they require. They complement traditional in-home caregivers and give family members extra peace of mind.
